Overview

Social Worker independently assesses residents from a clinical and psychosocial perspective, provides group interventions, and makes professional judgments of clinical and psychosocial needs. Drafts the multidisciplinary treatment plan with input from all disciplines. The social worker advocates for residents. This position provides individualized care and treatment that is consistent with age and life span developmental needs of each person and develops biopsychosocial summaries, conducts group and individual sessions, and coordinates treatment team meetings. Provides crisis intervention and assists residents in understanding the complexities of mental illness and utilizes resident contact as opportunities for empowering therapeutic interventions.

Qualifications

Education for Social Worker: 

  • Master’s Degree in Social Work or Mental Health Counseling .

Experience for Social Worker: 

  • Two (2) years’ experience with the severe and persistent mentally ill is preferred. 

Licenses/Certifications for Social Worker: 

  • Licensed as a social worker or mental health counselor.

Responsibilities

Develops biopsychosocial summaries, develops and implements interventions for barriers to discharge.

Advocates and negotiates with internal and external statewide systems on behalf of residents consistent with professional conduct and standards. Independently provides all care activities and appropriately seeks direction for unusual situations.

Provides crisis intervention and assists residents in understanding the complexities of mental illness

Utilizes resident contact as opportunities for empowering therapeutic interventions. Consistently attends and represents clinical needs of residents during community meetings and recovery team meetings. Coordinates team meetings and authors care plan with input from the multidisciplinary team.

Provides counseling and facilitates therapeutic groups in accordance with resident’s recovery plan objectives and needs. Provides information and support to family and significant others with the resident’s consent.

Maintains medical records in compliance with policies and accreditation standards. Updates all discharge information and documents direct and collateral contacts into medical record.
